Driver's License or Identification Card Social Security Card CPR Card (American Heart Association, American Safety & Health Institute, or Red Cross) Level 2 Background HIV/Aids Certificate (2 hours) Assisting with Self-Medication CEU (2 hours) Alzheimer's CEU (3 hours) Auto Insurance Policy (if willing to transport clients)
