Duties Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and observe patients' conditions closely, recording vital signs and changes in health status while maintaining detailed medical documentation within Electronic Health Record (EHR) systems. Administer medications and perform catheter care following home healthcare regulations and medical terminology standards under the supervision of healthcare professionals. Support patients with memory care needs, Alzheimer's disease, dementia, and end-of-life care by creating a safe, comforting environment tailored to individual needs. Assist with meal preparation, light cleaning, laundry, and other household tasks to promote a clean and healthy living space. Communicate effectively with clients and their families about care plans, progress notes, and any concerns while maintaining strict patient confidentiality in accordance with HIPAA guidelines. Respond promptly to emergencies by applying first aid or basic life support (BLS), including CPR when necessary, ensuring safety at all times. Qualifications Valid certification as a Home Health Aide or Certified Nursing Assistant with current licensing as required by state law. Proven experience working with geriatric patients, individuals with disabilities, or those requiring hospice or palliative care. Knowledge of medical terminology, vital signs measurement, medication administration, catheter care, and patient transfer techniques such as Hoyer lifts. Familiarity with EMR/EHR systems for accurate documentation of patient care activities. Strong understanding of home health care regulations, patient personal care assistance, and caregiving best practices. Ability to work independently in clients' homes while demonstrating excellent communication skills and compassion. Physical capability for heavy lifting and performing tasks that require mobility support or physical endurance.
