Hospice Nursing Assistant The Hospice Nursing Assistant provides personal care under the supervision of a nurse according to the patient's plan of care. This position will report to the Clinical Director of Hospice.

How to make an impact:

Provide personal care and assist with ADL's according to the Plan of Care - POC.Provide safe environment for patients.

Safely ambulate/transfer patient as appropriate.

Help with ROM, both active and assistant active exercises as directed by the nurse.

Prepare nutritional snacks as appropriate for the patient.

Under the direction of the nurse, perform simple, skin care in accordance to the plan of care where appropriate.

Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.

Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.

Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.

Attend and actively contribute during patient care conference.

Participate in Hospice discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.

Participate and/or attend in-services training to obtain 12 education hours per year.

Participate in the on-call rotation as scheduledAssist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)Demonstrate positive customer-service responsiveness according to agency standards.

Flexible in meeting patient/family needs that require variable shifts and weekends.

Participate in chart review quality improvement activities and other meetings as assigned.

Participate in communication with volunteer staff. What you'll bring to the table: Experience in hospice, medical facilities, home care or nursing homes, preferred.

Hospice/Palliative Care experience preferred.

Must be state tested certified nursing assistant in good standing and have 12 months experience as a nursing assistantKnowledge of community resources.
