We are seeking a dedicated and compassionate HHA/CNA Caregiver to provide exceptional home healthcare services to our clients. The ideal candidate will possess a strong background in patient care, with experience working with individuals of all ages, particularly seniors and those with developmental disabilities or memory care needs. As a vital member of our care team, you will deliver personalized assistance, ensuring safety, comfort, and dignity for each patient while adhering to all healthcare regulations and standards. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and observe patients' health status by recording vital signs, noting changes in condition, and communicating updates to healthcare professionals. Administer medications accurately following medical directives and document medication administration within electronic health record (EHR) or electronic medical record (EMR) systems. Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including engaging in therapeutic activities and maintaining a safe environment. Support end-of-life care by offering compassionate emotional support and ensuring comfort measures are followed in hospice settings. Maintain patient confidentiality in compliance with HIPAA regulations while managing medical records and documentation accurately. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Collaborate effectively with multidisciplinary teams to coordinate home & community care services tailored to individual needs. Qualifications Proven experience working as a Home Health Aide (HHA), Certified Nursing Assistant (CNA), or similar caregiving roles within home healthcare or long-term care facilities. Knowledge of medical terminology, medication protocols, catheter care, and basic life support (BLS) including CPR certification. Experience working with geriatric patients, individuals with disabilities, or those requiring dementia or Alzheimer's care. Familiarity with home health agency operations, home health regulations, and documentation procedures using EMR/EHR systems. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts when necessary. Strong communication skills for client interaction and team collaboration while maintaining patient privacy and confidentiality. Certification in first aid is preferred; ongoing training in hospice care or advanced caregiving techniques is advantageous.
