Job Description
Job Overview We are seeking a compassionate and dedicated Personal Care Worker / Certified Nursing Assistant (CNA) to provide exceptional support and care to our residents. In this vital role, you will assist individuals with their daily activities, ensure their safety, and promote their overall well-being. Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods such as Hoyer lifts or other equipment. Monitor patient observations regularly, recording vital signs such as blood pressure, pulse, respiration, and temperature, while maintaining accurate medical documentation in Electronic Health Record (EHR) systems. Administer medications following home healthcare regulations and medical terminology guidelines, ensuring proper documentation and adherence to HIPAA confidentiality standards. Support residents with specialized care needs such as dementia care, Alzheimer's care, hospice support, end-of-life assistance, and memory care activities. Conduct patient personal care assistance with compassion while maintaining patient confidentiality and respecting individual dignity. Utilize patient transfer methods safely during patient movement or repositioning; operate equipment like Hoyer lifts or wheelchair transfers as needed. Provide basic first aid and CPR in emergency situations, ensuring safety for residents and staff at all times. Assist with meal preparation, feeding assistance, laundry, cleaning, and maintaining a clean and safe environment for residents. Communicate effectively with clients and team members about care plans, changes in condition, or concerns related to resident health or safety. Skills Proven experience working with individuals with disabilities or in assisted living environments; geriatric or long-term care experience highly preferred. Strong knowledge of medical terminology, vital signs monitoring, medication administration, catheter care, and hospice or dementia-specific needs. Familiarity with EMR/EHR systems for medical documentation; understanding of home health care regulations and HIPAA compliance. Ability to perform heavy lifting safely using proper techniques; experience with patient transfer methods such as Hoyer lifts or gait belts. Excellent communication skills for engaging with patients empathetically while maintaining professional boundaries; experience working with geriatric patients is a plus. Knowledge of basic life support (BLS), first aid procedures, CPR certification preferred; ability to respond quickly in emergency situations. Capable of managing multiple tasks including meal prep, cleaning, laundry, and client communication efficiently within a home health setting. Demonstrated reliability in live-in employment scenarios or home & community care environments; understanding of home healthcare regulations is advantageous.
