Job Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Caregiver, where your dedication and skills will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized patient care, assist with daily activities, and ensure a safe, comfortable environment for individuals across various care settings. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and observe patients' conditions by performing patient observation, recording vital signs, and reporting changes promptly to healthcare professionals. Administer medications and treatments following medical documentation protocols, ensuring compliance with home healthcare regulations and HIPAA confidentiality standards. Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and end-of-life assistance. Maintain accurate medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; document patient care activities thoroughly. Prepare nutritious meals and assist with meal preparation while adhering to dietary restrictions and safety guidelines. Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment to promote overall health and well-being. Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring long-term or home health care. Strong knowledge of medical terminology, patient personal care assistance, catheter care, and vital signs measurement. Familiarity with home health agency operations, hospice services, and community-based caregiving practices. Ability to communicate effectively with clients and team members while maintaining patient confidentiality under HIPAA standards. Competence in using EMR/EHR systems for medical documentation and client communication. Knowledge of CPR, First Aid, Basic Life Support (BLS), and other emergency response procedures. Physical stamina for heavy lifting, patient transfer methods such as Hoyer lifts or other assistive devices, and working in diverse settings including assisted living facilities or private homes. Experience working within home healthcare regulations and providing compassionate end-of-life or hospice support when needed. Join us in delivering exceptional care that enriches lives!
