Job Summary Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA), where you will play a vital role in providing high-quality, patient-centered care to individuals across various care settings. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document patient vital signs such as blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems Observe patient conditions closely, noting any changes in behavior or health status for timely reporting Support medication administration following proper medical terminology and home healthcare regulations Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and dementia-specific activities Maintaine comfort and safety Prepare meals, assist with feeding when necessary, and maintain cleanliness of patient living areas including laundry and general cleaning tasks Experience Prior experience working in assisted living facilities or nursing homes is highly preferred Demonstrated experience caring for geriatric patients or individuals with developmental disabilities Knowledge of medical terminology, medication administration procedures, and HIPAA privacy standards Hands-on experience with electronic time login and documentation Familiarity with home health agency operations, home & community care regulations, and use of equipment such as Hoyer lifts Experience providing end-of-life or hospice care in compliance with safety standards like first aid and CPR certifications Ability to perform heavy lifting safely and handle physically demanding tasks associated with caregiving roles

Qualifications:

3-5 years caregiving experience in various settings including home care CNA certification a plus or PCA TB skin test / Chest x-ray within a year CPR and First Aid Reliable motor vehicle Able to pass a background check Valid ID Valid Social Security Card Join us to make a meaningful difference in the lives of those we serve!
