Job Description
Overview Join our dynamic healthcare team as a Float Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA), where your versatility and compassion will make a meaningful difference across multiple care settings. In this vital role, you will provide essential patient care, support clinical teams, and ensure a high standard of comfort and safety for diverse patient populations. Your adaptability will help us deliver seamless, quality care in long-term care facilities, hospitals, assisted living communities, and community-based programs. This position offers an exciting opportunity to expand your skills while contributing to the well-being of individuals with varying healthcare needs. Duties Deliver direct patient care including vital signs, hygiene assistance, mobility support, and medication reminders in accordance with care plans. Assist with patient transfers using equipment such as Hoyer lifts and ensure safe manual handling practices. Support clinical staff by preparing treatment areas, maintaining medical documentation, and managing electronic health records (EHR/Epic systems). Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments through compassionate engagement and behavioral health techniques. Participate in infection control protocols including aseptic technique and infectious disease precautions to maintain a safe environment. Collaborate with interdisciplinary teams to coordinate patient transfers, discharge planning, and community-based services. Maintain confidentiality in compliance with HIPAA regulations while ensuring accurate medical records management and documentation. Requirements Current certification as a CNA or PCA with active state licensure or registration. Experience working with geriatric patients, individuals with developmental disabilities, or those requiring memory or dementia care. Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; proficiency in medical documentation is preferred. Knowledge of vital signs measurement, medication administration support, catheter care, and basic life support (BLS). Ability to perform heavy lifting safely using proper lifting techniques; manual handling skills are essential. Strong communication skills to effectively coordinate with team members and caregivers. Prior experience in long-term care facilities, hospitals (including ICU or trauma centers), hospice or home care settings is highly desirable.
