Adult Primary Care (Ages 18+) About the Role We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing primary care team. The ideal candidate will provide high-quality, patient-centered care to adults ages 18 and above while managing acute and chronic conditions across commercial insurance, Medicare, Medicaid, and Medicare Advantage/value-based care populations. Experience with eClinicalWorks (eCW) is preferred but not required. Key Responsibilities Provide comprehensive primary care to adult patients (18+) in an outpatient setting. Diagnose, treat, and manage both acute and chronic medical conditions. Perform detailed patient assessments; order, review, and interpret diagnostic tests. Develop individualized, evidence-based treatment plans. Identify and close HEDIS and quality-of-care gaps. Document all care accurately in the EMR (eClinicalWorks preferred but not required). Collaborate with physicians, medical assistants, and care coordination teams. Educate patients on disease prevention, medications, and self-management. Support population health and value-based care initiatives. Promote a positive patient experience and uphold high standards of care. Qualifications Valid and unrestricted Nurse Practitioner (NP/APRN) license or Physician Assistant (PA) license in the State of Georgia. National certification as a Nurse Practitioner, such as FNP, AGNP, or equivalent, or Physician Assistant certification, such as
PA-C/NCCPA
certification. 1-2 years of primary care experience preferred but not required. Experience working with Medicare, Medicaid, Medicare Advantage, and commercial plans preferred but not required. EMR proficiency; eClinicalWorks experience preferred but not required. Strong clinical assessment, critical thinking, and communication skills. Ability to work collaboratively in a team-based environment. Ability to meet credentialing, payer enrollment, and collaborative/supervisory requirements as applicable to role and licensure.
