Job Description

This position is eligible for a $2,000 sign-on bonus. Details in Compensation & Benefits section below. Job Summary The Clinic Support Worker (CSW) is a care team member who supports the delivery of healthcare by providing clinical care and administrative support as delegated by professional clinical staff. The CSW follows up on patient identified goals, identifying and matching patient needs with community resources, providing interpretation and translation services, and advocating for culturally diverse patients. Coordinates PHC team members to engage patients, their families, and the community in the patient's care; to provide positive experiences for patients; and to serve as a resource for staff and patients about community resources. Performs tasks which do not require a license, professional judgment, or intervention. Essential Functions Performs basic clinical duties such as rooming patients, obtaining vital signs, height, weight, and collection of basic patient intake questions and data in the clinic or on the telephone as assigned. Participates in initial data gathering on patients and goals using tools in the EMR and other tools such as PRAPARE, the PHQ-9 and other tools as instructed.. In conjunction with other team members, responsible for follow up on identified issues, such as: referrals for other services, assisting patients in setting and keeping appointments, setting expectations for patients about their follow up appointments, clarifying and ensuring comprehension of care instructions and serving as a liaison to ensure patients are connected and engaged with their care team. Provides interpreting and/or translating services as requested. As directed by the professional clinical staff, reports normal results and pertinent information to patients. Maintains documentation of patient contacts in compliance with PHC policies. Follows organizational procedures/protocol for the organization and management of the electronic medical record (i.e. test results, retrieval, filing, creation, repair, changing of electronic medical record). Acts to facilitate interdisciplinary communication. Utilizes other members of the health care team in assessing and analyzing patient data. Assesses learning needs of the patient and significant others. Gives accurate and complete information to patient and family. Provides assistance to patients and their families to identify needs and facilitate access to community resources and entitlements. . Refers patients to the Health Benefit Specialist when appropriate. Responds to referrals for assistance from and facilitates referrals to PHC clinics, community agencies, and other resources. Prepares exam rooms in accordance with established infection control protocols. Provides cross coverage for other clinic functions within the scope of the position and for which competency has been determined. Maintains supply inventory and stocks supplies in various work areas. Demonstrates current level of knowledge of various payor regulations by functioning within those guidelines. Utilizes appropriate procedures, including the use of PPE (Personal Protective Equipment) in performance of patient care duties. Characteristic Duties of All Primary Health Care Employees Supports a service oriented atmosphere. Improve work processes and clinical outcomes including disparity and quality improvement initiatives. Follows PHC's policies and procedures and reports needed changes. Develops and maintains own competence including completion of all assigned training within assigned deadlines. Establishes and maintains effective working relationships with team members. Maintains a safe working environment and practices safe working habits. Protects patients' rights by maintaining patient confidentiality per HIPAA and the PHC Corporate Compliance Program. Participates in cross training and works as a team to facilitate patient care. Attends department meetings, training programs and committee meetings as requested. Consistently meets assigned work schedule. Leads by example, demonstrates PHC's iCare values in daily work. Performs other duties as assigned.
Minimum Qualifications Education:
High School diploma or equivalent.
Experience:
One year of experience in human services, healthcare, interpretation, translation, or related field
License/Certifications:
Basic Life Support (BLS) for Health Care Providers Certification
  • Mandatory Reporter Certification
  • Must be obtained with Introductory Period if not current.
Knowledge, Skills or Abilities:
Effective English & other specified language (i.e. Spanish, Burmese, Nepali, etc.) verbal and written communication skills Proficiency with computers, including Microsoft Word, Excel and Powerpoint, and ability to learn and effectively use software applications, including practice management systems and electronic medical records Ability to assist patients input data into an online program Knowledge of community resources, including entitlement programs. Ability to translate language to written form. Ability to interact in a calm and empathetic, yet professional and efficient manner Ability to establish and maintain effective working relationships
Other Job Requirements:
Consistently meets work schedule as designated Must be at least 18 years old Typical Work Hours This position typically works 40 hours per week, Monday 8 AM
  • 8 PM, Tuesday & Wednesday 8 AM
  • 5 PM, Thursday 8 AM
  • 8 PM, Friday Off. This position participates in the Saturday rotation, which is 9 AM
  • 1 PM approximately every 5th week.
Compensation & Benefits Compensation is dependent on education and experience. PHC offers a comprehensive benefits package, including: Eligible for $2,000 sign-on bonus payable as $1,000 at hire, $500 at 6 months, and $500 at 1 year Qualified individuals are eligible for up to a $500 quarterly retention bonus as part of special funding through the American Rescue Plan Act (ARPA). Bonuses are time limited through the Spring of 2023.
