Job Description

To provide Restorative Nursing care to the residents of the facility safely and keeping in mind the goal of maintaining or improving their functional mobility. Perform Safe Ambulation, Transfers and Range of Motion (ROM), assist with activities of daily living and perform other related duties as assigned by supervisor. PT OT Nursing ST Duties and Responsibilities:
Work under the direction of the Restorative Nurse/Director of Nursing Work with restorative nurse to assess ADL abilities of residents newly admitted to facility, recently discharged from therapy, and with every MDS assessment. Collaborate with RN to create a care plan for each resident in the restorative program. Collaborate with RN to revise and update care plans with any change in the resident's abilities and with every restorative assessment. Attend all care plan meetings. Work closely with Therapy department by: Following programs as prescribed Consulting with Therapy department as needed Instruct nursing assistants in proper procedures. Assist and instruct residents with daily ROM exercises and restorative exercises as outlined in each resident's personalized care plan. Chart restorative cares in flow sheet and add notes for any exceptions, special circumstances, or additional information in restorative notes. Collaborate with restorative nurse to evaluate the restorative potential of every resident who has a fall. Attend facility meetings Assist in getting residents dressed and ready for the day before breakfast and helping residents to and from meals as time allows when not doing restorative exercises with other residents
Education/Training:
Equivalent of Completion of High School C N A certification Qualifications Must be Certified Nurses Assistant Must have CPR certification
Development and Participation:

  • Ability to communicate with personnel, patients, and residents
