Acute Rehabilitation Job Summary:
The Certified Nursing Assistant (CNA) provides compassionate, individualized patient care and supports the nursing and rehabilitation team in helping patients regain function and independence. The CNA assists with daily patient care needs, monitors and reports patient conditions, and contributes to a safe and supportive rehabilitation environment. The current facility posting specifically notes that CNAs assist rehabilitation nurses with delegated rehabilitation treatment and patient care according to applicable regulations and guidelines.
Key Responsibilities:
Provide direct patient care under the direction of nursing staff. Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, and mobility. Assist with transfers, positioning, ambulation, and other mobility needs. Take and document vital signs and report changes in patient condition to the nursing team. Assist rehabilitation nurses with delegated treatments and patient-care activities. Maintain patient safety and follow infection-control and facility procedures. Communicate effectively with patients, families, nurses, and the rehabilitation team. Support patients in achieving their rehabilitation goals and increasing independence. Maintain accurate and timely patient-care documentation. Provide compassionate, respectful care to patients with varying levels of acuity and mobility.
Ideal Candidate:
Active/current Illinois CNA certification . Experience in an acute-care hospital, inpatient rehabilitation, or similar clinical setting preferred. Strong patient-care and communication skills. Ability to work collaboratively with nurses and rehabilitation professionals. Comfortable assisting patients with mobility and activities of daily living.
