Monday - Friday 8 hour shifts
Salary Range:

What You'll Do As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families.
Your responsibilities include:

Requirements:
What You Bring Active CNA License or Certification in the state of employment Possess and maintains current CPR certification One (1) + year Hospice, home health or related organization experience preferred Comfort and sensitivity working with terminally ill patients and their families A calm, dependable presence under emotionally demanding circumstances Strong attention to detail in observation and documentation A valid driver's license and willingness to travel locally for patient visits
