We work as an extension of the hospital consortiums you trust in your community, with the goal of improving care across the continuum for our homebound patients. Join our team and make a difference in your community serving Hamilton county and the surrounding areas. Provides personal care activities of daily living such as bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing. Communicates changes in patient status to the supervising Registered Nurse (RN) in a timely manner. Reports patient complaints to the RN Creates and submits legible, accurate and timely documentation. Performs duties in compliance with agency policies and procedures and local, state, and federal regulations. Demonstrates attention to details for patient's safety and safety of their home environment. Reports unsafe conditions in the home. Practices effective infection control and adheres to infection control policies and procedures. Practices safe lifting and transferring of patients. Communicates clearly and positively with patients, families, team members and others. Displays a caring, patient's first behavior. Keeps patients and/or family informed regarding visit time and schedule changes. Attends in-service programs to meet compliance requirements Maintains patient confidentiality in compliance with HIPAA Basic Qualifications Must be a Certified Nursing Assistant (CNA) and/or possess current Home Health Aide registrar in the state of Indiana. Must have high school diploma or equivalent Must possess valid driver's license and automobile insurance in the state of Indiana Must be able to provide own transportation to assignments.
