Responsibilities Provide direct patient care by assisting with activities of daily living such as bathing, dressing, feeding, and mobility support using appropriate patient transfer methods and manual handling techniques. Monitor vital signs and observe changes in patient conditions, reporting findings accurately in electronic health records (EHR) systems and medical documentation. Support patients with specialized needs including dementia care, memory care, hospice care, and behavioral health requirements. Administer medications under supervision and ensure medication administration protocols are followed in accordance with HIPAA regulations and safety standards. Utilize equipment such as Hoyer lifts and assistive devices to ensure safe lifting practices during patient transfers or repositioning. Maintain a clean and safe environment by adhering to aseptic techniques, infection control procedures, and sanitation guidelines. Communicate effectively with patients, families, and interdisciplinary team members to promote a supportive care environment. Assist with medical procedures such as phlebotomy or catheter care when trained and authorized to do so. Document all patient interactions accurately within EMR/EHR systems while maintaining strict confidentiality of medical records. Experience Prior experience working as a Certified Nursing Assistant in settings such as long-term care facilities, hospitals (including ICU or trauma centers), assisted living communities, or home care is highly desirable. Demonstrated knowledge of medical terminology, anatomy knowledge, physiology principles, and basic life support (BLS) certification is required. Experience working with geriatric patients or individuals with disabilities—including developmental disabilities—is preferred. Familiarity with electronic health records (EHR) systems such as Epic, Cerner, or eClinicalWorks is advantageous. Exposure to specialized areas such as Alzheimer's/dementia care, hospice services, pediatric or neonatal units (PICU), or surgical environments enhances candidacy. Ability to perform heavy lifting safely and adhere to safe lifting practices during patient handling is essential. Proven ability to communicate effectively within multidisciplinary teams while maintaining patient confidentiality under HIPAA guidelines.
