Opportunities Compassionate Care Plus is currently hiring both Registered Nurses (RNs) and Certified Nursing Assistants (CNAs) for our Home Hospice Program. Our Home Hospice team provides compassionate, patient-centered care wherever our patients call home, including private residences, nursing homes, and assisted living facilities. These roles offer independence and flexibility in the field while still providing the support of an experienced interdisciplinary hospice team.

REGISTERED NURSE
(RN) - HOME HOSPICEAbout the Role The Home Hospice RN works as part of the hospice interdisciplinary team to ensure patients and families have continued access to compassionate, supportive care throughout the end-of-life journey. This position serves as a float RN , assisting primary nurses throughout the counties we serve. The RN carries out the established plan of care, addresses identified physical, psychosocial, and other patient needs, and provides education and support to patients and families. Our nurses enjoy autonomy in managing their daily schedules while having the support of the entire hospice team. RNs are provided with an iPhone and tablet and receive mileage reimbursement. Leased vehicles may also be available. RN Qualifications Current Kentucky license as a Registered Nurse Minimum of one year of nursing experience; two years preferred Current CPR certification Experience in pain and symptom management preferred Experience caring for terminally ill patients preferred Ability to work effectively as part of an interdisciplinary team Strong communication and interpersonal skills with patients, families, physicians, community partners, and team members Current Kentucky driver's license and reliable access to an appropriately insured vehicle Ability to lift and/or move a minimum of 40 pounds using proper body mechanics and safety measures; individual responsibilities may occasionally require lifting more than 40 pounds Ability to perform the physical requirements of the position, including sitting, standing, walking, seeing, hearing, speaking, and using hands to handle equipment and supplies
CERTIFIED NURSING ASSISTANT
(CNA) - HOME HOSPICEAbout the Role The Home Hospice CNA works in conjunction with the patient's primary nurse to provide compassionate personal care in accordance with the nursing assignment and established plan of care. Our CNAs have the opportunity to build meaningful relationships with patients and families while working independently in the community with the support of their team nurse and the entire interdisciplinary hospice team.
Responsibilities include:
