The Certified Nursing Assistant (CNA) / Patient Care Technician (PCT) at Serenity Oaks IRF provides essential direct patient care services under the supervision of the Registered Nurse, contributing to the comfort, safety, and functional progress of rehabilitation inpatients. This role assists patients with activities of daily living, monitors and reports changes in patient condition, and reinforces functional skills taught by the therapy team. The CNA/PCT is a vital member of the care team and interacts with patients and families more frequently than almost any other clinical role. 2.

ESSENTIAL FUNCTIONS & RESPONSIBILITIES

Assist patients with ADLs including bathing, oral hygiene, grooming, dressing, and toileting under RN supervision. Assist patients with mobility, transfers, and ambulation using proper body mechanics and assistive devices as directed. Obtain and record vital signs, weight, and intake/output measurements; report abnormal findings to the supervising RN promptly. Answer call lights promptly; respond to patient needs in a courteous, respectful, and timely manner. Reinforce functional techniques and strategies taught by occupational therapy, physical therapy, and speech therapy. Maintain a safe, clean, and organized patient room and environment. Apply and remove antiembolism stockings, splints, and positioning devices as directed by the RN. Transport patients to therapy, diagnostic areas, and other areas of the facility as needed. Observe and report changes in patient skin condition, behavior, appetite, and functional status to the supervising RN. Assist with meal delivery, feeding assistance, and fluid intake encouragement. Document care provided and observations in the EHR as authorized and trained. Participate in rounding with the nursing team and report patient concerns or needs. Maintain confidentiality of all patient information and adhere to facility privacy policies. Participate in facility education, competency training, and quality improvement activities. 3.

EDUCATION & EXPERIENCE REQUIREMENTS

Required:

High school diploma or GED.

Required:

Completion of a state-approved nurse aide training program.

Preferred:

Six (6) or more months of CNA/PCT experience in an acute care, rehabilitation, or long-term care setting. 4.

LICENSURE, CERTIFICATION & REGISTRATION REQUIREMENTS

Required:

Current listing on the Louisiana Nurse Aide Registry (maintained by the Louisiana Department of Health).

Required:

No adverse findings (abuse, neglect, or misappropriation) on the Louisiana Nurse Aide Registry.

Required:

Current BLS/CPR certification (AHA or Red Cross).

Required:

Registry status must be verified prior to commencement of employment and monitored throughout employment. 5.

KNOWLEDGE, SKILLS & ABILITIES

Knowledge of basic patient care and assistance techniques for rehabilitation populations. Understanding of body mechanics, safe patient handling, and use of assistive transfer devices. Ability to measure and accurately record vital signs, weight, and intake/output. Effective communication skills for interacting with patients, families, and the nursing team. Sensitivity to the physical and emotional needs of patients in a rehabilitation setting. Knowledge of infection control principles and standard precautions.
