Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare professionals. Administer medications and perform catheter care following prescribed medical protocols while maintaining strict adherence to home health care regulations. Document medical information accurately using Electronic Health Record (EHR) systems or Medical Records software, ensuring compliance with HIPAA and patient confidentiality standards. Support patients with meal preparation, light cleaning, laundry, and other household tasks to promote a safe and comfortable environment. Provide specialized dementia and Alzheimer's care, memory care, hospice support, and end-of-life assistance with compassion and respect. Communicate effectively with clients and their families to ensure understanding of care plans and address individual needs with sensitivity. Requirements Valid CNA certification or HHA license required; prior experience in nursing homes, assisted living or home healthcare strongly preferred. Demonstrated experience working with individuals with developmental disabilities or geriatrics. Knowledge of medical terminology, medication administration, medical documentation, and vital signs monitoring. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Certification in First Aid and Basic Life Support (BLS), including CPR; ongoing training preferred. Familiarity with EMR/EHR systems for accurate recordkeeping; understanding of home health agency operations is a plus. Strong communication skills for effective client interaction and team collaboration. Commitment to maintaining patient confidentiality in accordance with HIPAA regulations. Ability to work independently in home settings or within community-based programs; flexibility for live-in employment or varied shifts is advantageous.
