Job Overview Join our dedicated healthcare team as a Certified Nursing Assistant (CNA) / Home Health Aide (HHA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates, ensuring timely communication of any concerns. Support medication administration following medical records protocols while maintaining strict adherence to HIPAA regulations to protect patient confidentiality. Observe patients closely for changes in condition or behavior, especially in memory care or dementia care settings, and document findings in electronic health record (EHR) systems. Provide specialized care for individuals with developmental disabilities, Alzheimer's disease, or end-of-life needs while maintaining a compassionate approach aligned with hospice or palliative care standards. Assist with medical procedures such as catheter care and basic life support (BLS), including CPR when necessary. Perform household tasks like cleaning, laundry, meal preparation, and ensuring a safe environment that complies with home health care regulations. Communicate effectively with clients and their families about care plans and progress to foster trust and clarity in service delivery. Support clients during transfers using proper patient transfer methods or equipment like Hoyer lifts or gait belts to prevent injury. Maintain accurate medical documentation and ensure all client records are up-to-date per legal requirements. Experience Previous experience working as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred. Demonstrated knowledge of medical terminology, medication administration procedures, and vital signs monitoring. Proven ability to provide compassionate patient personal care assistance across diverse populations including geriatrics and individuals with disabilities. Experience working with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's is advantageous. Familiarity with EMR/EHR systems for documentation purposes is desirable. Knowledge of home & community care standards along with home health regulations ensures compliance at all times. Ability to perform heavy lifting safely when required and operate patient transfer equipment confidently. Certification in First Aid and CPR is essential; additional certifications like Basic Life Support (BLS) are a plus. $23.00 - $25.00 per hour Expected hours: 12.0 - 40.0 per week
