Duties Observe and monitor patients' conditions regularly, noting any changes or concerns with precision and professionalism Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations and medical documentation standards Take vital signs including blood pressure, pulse, respiration, and temperature to ensure patient stability and report findings accurately Support patients with medical needs such as catheter care, wound care, dementia or Alzheimer's care, and end-of-life assistance in compliance with home health care regulations Document patient information thoroughly in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems and update medical records diligently Maintain a clean and safe environment by assisting with laundry, cleaning, meal preparation, and ensuring infection control practices are followed Qualifications Valid CNA certification or equivalent caregiving credentials required; experience working with geriatric patients or individuals with disabilities is highly valued Proven experience in assisted living facilities, nursing homes, hospice or home healthcare settings preferred Knowledge of medical terminology, medication administration procedures, and vital signs monitoring essential for effective patient care Familiarity with EMR/EHR systems and medical documentation practices necessary for accurate recordkeeping Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts when needed Strong communication skills to effectively interact with patients, families, and healthcare teams while maintaining confidentiality under HIPAA guidelines Certification in First Aid and Basic Life Support (BLS) or CPR required; additional certifications in dementia or Alzheimer's care are a plus Join us in providing compassionate caregiving that transforms lives!
