Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility assistance. Provide companionship and emotional support while promoting client independence. Prepare meals and assist with feeding when needed. Perform light housekeeping, laundry, and other homemaking tasks. Safely transfer and reposition clients using proper body mechanics and equipment such as Hoyer lifts, gait belts, and transfer devices. Observe and report changes in a client's condition to the office promptly. Accurately document visits using our Electronic Visit Verification (EVV) system and mobile application. Follow each client's individualized care plan while maintaining HIPAA confidentiality. Provide care for clients with Alzheimer's disease, dementia, Parkinson's disease, hospice needs, and other specialized conditions. Maintain a clean, safe, and comfortable home environment. Communicate professionally with clients, families, nurses, and office staff. Requirements Active HHA, CNA, or PCA certification preferred. Previous home care, assisted living, nursing home, or hospice experience preferred. Current CPR certification is a plus. Experience using Hoyer lifts, gait belts, and safe transfer techniques strongly preferred. Must be able to lift, push, pull, and safely transfer up to 50 pounds independently and more with proper lifting equipment or assistance. Comfortable using a smartphone for EVV clock-in/clock-out and documentation. Excellent communication and time-management skills. Reliable transportation.
