Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration Administer medications and perform catheter care following medical protocols and home healthcare regulations Support patients with dementia, Alzheimer's, and memory care needs through specialized care techniques Document patient information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Facilitate safe patient transfers utilizing proper transfer methods and equipment such as Hoyer lifts or patient transfer belts Maintain patient confidentiality in compliance with HIPAA regulations while ensuring respectful and compassionate communication Provide end-of-life and hospice care with sensitivity, dignity, and respect for patient wishes Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Follow all safety procedures including first aid, CPR certification standards, and proper lifting techniques for heavy lifting tasks Skills Proven experience working with geriatric patients and individuals with disabilities or developmental disabilities Strong knowledge of medical terminology, medication administration, and vital signs monitoring Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Excellent patient observation skills to detect changes in condition or behavior promptly Ability to communicate effectively with clients, families, and healthcare team members Experience providing dementia or Alzheimer's care within memory or senior care settings Knowledge of home health care assistance regulations and best practices for home & community care environments Ability to perform basic life support (BLS), first aid, CPR confidently and consistently follow safety protocols Physical stamina for activities involving heavy lifting, patient transfers, and extended periods of standing or walking Compassionate caregiving attitude combined with professionalism to uphold patient privacy and confidentiality at all times Join us in delivering exceptional home health care that enriches lives through dedicated service.
