Job Description

Overview We are urgently seeking compassionate and dependable Certified Nursing Assistants (CNAs), Personal Care Assistants (PCAs), Home Health Aides (HHAs), and Caregivers to join our team for per-diem home health care assignments. In this rewarding role, you will provide personalized, one-on-one care to clients in the comfort of their homes, helping them maintain their independence, safety, and quality of life. Note:
The position requires traveling with the client, so candidates must have a reliable personal vehicle and a valid driver's license. Duties Provide exceptional in-home care by assisting clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, meal preparation, and mobility assistance. Monitor and document client conditions, vital signs, and daily activities, reporting any changes or concerns to the supervising nurse or care team. Assist clients with medication reminders and support care plans as directed by healthcare professionals. Provide companionship and emotional support to promote client well-being and reduce social isolation. Support clients with Alzheimer's disease, dementia, memory care needs, developmental disabilities, or other chronic health conditions. Assist with safe transfers, ambulation, and mobility using appropriate equipment, including gait belts, walkers, wheelchairs, or Hoyer lifts when required. Maintain accurate documentation and ensure compliance with company policies, care plans, and HIPAA privacy regulations. Follow infection control and safety protocols to provide a clean and safe home environment. Assist with light housekeeping tasks related to client care, including laundry, organizing living spaces, and maintaining a safe environment. Schedule Consistent schedule with recurring shifts Qualifications Current Certified Nursing Assistant (CNA), Personal Care Assistants (PCA), Home Health Aide (HHA) certification, or equivalent caregiving credential, as required by state regulations. Previous home health care, private duty, or caregiving experience preferred. Knowledge of basic patient care techniques, vital signs monitoring, and infection control practices. Ability to follow individualized care plans and accurately document client information. Strong communication, interpersonal, and organizational skills. Demonstrated compassion, professionalism, and commitment to providing high-quality client care. Experience caring for seniors, individuals with dementia, Alzheimer's disease, disabilities, or chronic health conditions is highly preferred. Ability to safely assist with transfers, mobility support, and other physical caregiving duties. Reliable transportation and the ability to travel to and with the client homes as needed. Why Join Us?
