Job Description

We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to join our team. In this vital role, you will provide exceptional patient-centered care to individuals in their homes and other care settings. Your energy and enthusiasm will help improve the quality of life for our clients by assisting with daily activities, monitoring health conditions, and ensuring their comfort and safety. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods such as Hoyer lifts when necessary. Monitor patient health by taking vital signs such as blood pressure, pulse, respiration, and temperature; document medical information accurately using Electronic Health Record (EHR) systems. Support medication administration following home healthcare regulations and medical documentation protocols while maintaining patient confidentiality in accordance with HIPAA standards. Observe and report changes in patient condition, including signs of dementia or Alzheimer's disease, ensuring prompt communication with healthcare teams. Provide specialized care such as catheter care and dementia/memory care to meet individual needs. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Support patients during transfers or mobility exercises using proper techniques to prevent injury for both the patient and caregiver. Skills Proven experience working with geriatric patients or individuals with developmental disabilities in home care or nursing home settings. Strong knowledge of medical terminology, medical records management, and documentation standards. Skilled in patient observation techniques and recognizing changes in health status or behavior. Familiarity with EMR/EHR systems for accurate recordkeeping and communication within healthcare teams. Ability to perform CPR and First Aid confidently; certification preferred but not mandatory upon hire. Excellent client communication skills with a compassionate approach to caregiving. Physical stamina for heavy lifting, long shifts, and assisting with patient transfers using equipment like Hoyer lifts or other transfer methods.
