In this vital role, you will provide high-quality, patient-centered care to diverse populations across various care settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using proper manual handling techniques and equipment like Hoyer lifts. Monitor and record vital signs including blood pressure, pulse, respiration, and temperature accurately within electronic health record (EHR) systems. Support medication administration under supervision, ensuring adherence to HIPAA regulations and maintaining patient confidentiality. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral support. Assist with medical procedures such as catheter care, phlebotomy, and aseptic techniques in accordance with hospital or clinic protocols. Collaborate effectively with nursing staff and other healthcare professionals to ensure seamless patient service across inpatient, outpatient, or home & community care settings. Maintain detailed medical documentation and ensure compliance with all safety standards including infection control practices in high-acuity environments like ICU or trauma centers. Qualifications Valid GNA or CNA certification required; additional certifications such as Basic Life Support (BLS), CPR, or Phlebotomy certification are highly desirable. Prior experience working in critical care settings such as ICU, Level I Trauma Centers, or Emergency Medicine is preferred. Demonstrated experience with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for accurate documentation. Knowledge of medical terminology, anatomy, physiology, and patient transfer methods to ensure safe patient handling. Experience working with geriatric patients or individuals with developmental disabilities is a plus; familiarity with dementia and memory care is beneficial. Strong communication skills for caregiver interactions and patient service excellence; ability to work effectively within a multidisciplinary team. Physical ability to perform heavy lifting and safe manual handling practices; capable of standing for extended periods while providing attentive patient care.
