Overview We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs), Certified Medication Technicians (CMTs), and Home Health Aides (HHAs) to provide exceptional patient-centered care across various healthcare settings. Duties Provide direct patient care, including assistance with bathing, dressing, grooming, and mobility to promote comfort and independence. Monitor and record vital signs accurately using electronic health record (EHR) or electronic medical record (EMR) systems, ensuring precise medical documentation. Assist with medication administration following prescribed protocols while maintaining compliance with HIPAA regulations to protect patient privacy. Support patients with activities of daily living such as meal preparation, light cleaning, laundry, and maintaining a safe environment. Observe and report changes in patient condition, including symptoms related to dementia, Alzheimer's disease, or other memory impairments. Utilize specialized equipment such as Hoyer lifts for safe patient transfers and catheter care techniques as needed. Collaborate with nursing staff and other healthcare professionals to develop individualized care plans tailored to each patient's needs. Maintain a clean, organized workspace while adhering to infection control standards and safety procedures in hospice, long-term care, or home health settings. Qualifications Valid certification as a CNA, CMT, or HHA from an accredited program; prior experience in nursing homes or assisted living facilities is highly desirable. Knowledge of medical terminology, vital signs measurement, medication administration, and dementia or Alzheimer's care. Experience working with individuals with developmental disabilities or disabilities requiring specialized support. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. CPR certification is required; additional training in hospice or hospice care is advantageous. Ability to perform patient observation accurately and respond appropriately to urgent situations. Strong communication skills coupled with a compassionate approach to caregiving; ability to work independently or as part of a team in home & community care environments. Physical stamina for assisting with ADLs, transfers using Hoyer lifts, and providing comprehensive patient care across diverse settings.
