A Nursing Assistant provides hospital and nursing home patients with basic care and assists them with daily activities. Helps patients with dressing, washing, and feeding. May perform basic medical care under the direction of a nurse or other medical professional. These roles typically require completion of a regional agency-approved training program in order to obtain a certification before practicing.

As part of an interdisciplinary care team, we deliver personalized support that keeps our participants healthy, engaged, and at home where they want to be. Position Summary We are hiring 2 full-time Certified Nursing Assistants (CNA) / Field Aides to provide hands-on personal care and support to PACE participants in their homes throughout our service area, based out of Newaygo, MI. This is a rewarding opportunity to build meaningful relationships with the same participants over time while working as part of a mission-driven care team.
Service Area:
Newaygo, Mecosta, Montcalm, Osceola, Lake, Oceana, and Mason counties Key Responsibilities Provide personal care assistance including bathing, grooming, dressing, and toileting Assist with mobility, transfers, and use of assistive devices (e.g., Hoyer lifts, gait belts) Support light housekeeping, meal preparation, and medication reminders Monitor and report changes in participant condition to the care team Document care provided accurately and in a timely manner Travel to participants' homes throughout the service area (Newaygo, Mecosta, Montcalm, Osceola, Lake, Oceana, and Mason counties) Communicate professionally with participants, families, and the interdisciplinary team (IDT) Qualifications Current CNA certification (required) Prior home care, hospice, or long-term care experience preferred Valid driver's license and reliable transportation Ability to perform physical tasks including lifting and transferring participants Strong communication skills and a compassionate, patient-centered approach Comfortable working independently in participants' homes What We Offer Full-time schedule with 2 openings available Pay range: $18.00-$21.00/hour Mileage reimbursement for travel between participant homes Opportunity to be part of a supportive, mission-focused PACE care team Consistent caseload, allowing you to build lasting relationships with participants
