Duties Act as a primary point of contact for patients, families, and healthcare teams to facilitate effective communication and support throughout the care continuum. Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to inform personalized care plans Coordinate clinical case management activities such as discharge planning, post-acute care arrangements, and outpatient follow-up appointments Collaborate with surgeons, nurses, and other healthcare providers to develop and implement patient-centered treatment strategies Manage electronic health records (EHR) accurately and efficiently, ensuring compliance with HIPAA regulations and medical terminology standards Assist in diagnostic evaluations by gathering relevant medical records, supporting infectious disease care protocols, and monitoring patient progress in surgical settings Support utilization management efforts by evaluating patient needs against facility policies and managed care guidelines to optimize resource use. Qualifications Proven experience in critical care environments such as ICU, emergency medicine, or medical/surg nursing. Experience in perioperative and/or surgical nursing is preferred. Familiarity with EMR/EHR systems, medical terminology, HIPAA compliance, and health regulation policies Excellent patient service skills with the ability to communicate effectively with diverse populations. Demonstrated ability to perform patient assessments involving vital signs and physiology knowledge; experience managing patients as a nurse case manager is advantageous Ability to collaborate seamlessly with healthcare professionals across disciplines such as surgeons, nurses, and numerous clinical offices. Strong organizational skills for managing intake processes, discharge planning activities, diagnostic evaluations, and case management documentation.
