Make a meaningful difference every day as a Certified Nurse Aide (CNA) in our PACE Day Center, providing compassionate, hands-on care and support to frail elderly participants. Working closely with the healthcare team, you will help carry out individualized care plans that promote comfort, dignity, independence, and overall well-being in a warm, participant-centered environment. Under the guidance of the Day Center Supervisor, this role offers the opportunity to build meaningful connections while making a lasting impact in the lives of older adults. - Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan. Support Mealtime
- Help participants enjoy their meals and stay hydrated for good health. Keep Things Moving
- Support mobility, including turning, positioning, and safe transfers. Perform Vital Checks
- Measure and record vital signs, weight, and intake/output. Provide Responsive Support
