What You'll Do:
Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR Work independently, but never alone Travel to patient homes within assigned territory Designed for Success Dedicated field education team supporting you from Day 1 Leadership that listens, mentors, and champions your growth A collaborative culture where your impact is recognized and your feedback shapes improvement Benefits That Support Your Whole Life Pathway to
