Summary :
Under the supervision of the LPN, RN, or nurse leader, the Certified Nursing Assistant (CNA) performs patient-centered, evidence-based nursing care activities to meet the physical and psychosocial needs of the patient. Through collaborative practice with all members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Contributes to the goals of the department by being accountable for the delivery of compassionate, safe care and demonstrated commitment to continuous growth and professional development.Job Description:

Join our dedicated home care team and provide personalized care and support to patients in the comfort of their own homes. What You'll Do As a Home Care Certified Nursing Assistant (CNA), you'll help patients maintain their independence and quality of life by providing personal care and support services, including:- Assisting with bathing, grooming, and personal hygiene
- Supporting safe mobility, transfers, and ambulation
- Assisting with activities of daily living (ADLs)
- Monitoring and reporting changes in patient condition
- Providing companionship and emotional support
- Reinforcing care plans under the direction of the nursing and/or therapy team You'll work independently in patients' homes while being supported by a collaborative and caring team dedicated to delivering exceptional care. What We Offer

Monday-Friday, 8:00 AM-4:30 PM o 2 consistent days off per week- Flexible Scheduling
