Qualifications:
  • Current registration with state of Minnesota as Certified Nursing Assistant
  • Experience as a CNA/HHA is preferred
Primary Objective:
To provide compassionate, hands-on personal care to the elderly, ill or those facing end of life, ensuring their safety, comfort, dignity, and quality of life in their home setting.

Essential Functions:
  • Attendance and reliability for clients to receive uninterrupted services
  • Provides for personal hygiene needs including bathing, dressing, positioning, hair care, mouth care toileting, and bed changes.
  • Provides for body positioning, transfers, therapeutic exercises, active or passive range of motion.
  • Provides for skin care, including full or partial bathing and foot soaks.
  • Performs delegated nursing or therapy procedures including medication or assistance when: Instructed by RN or therapist in proper methods to perform procedure, RN or therapist specifies in writing instructions for performing procedure and has demonstrated competence.
  • Promptly reports any changes in client condition to RN case manager or RN supervisor
  • Home maintenance duties including laundry, bed making and cleaning of home environment
  • Provides assistance with meal prep and eating
  • Promotes mental alertness through conversation and other activities. Gives emotional and psychological support to client and family members.
  • Maintains and completes accurate documentation in a timely manner
  • Point of Care documentation will be completed with the expectation that at least 90% of the documentation is done in the patient's home.
  • Visits should be completed and synced in on same day as visit.
  • Attends mandatory in-services and completes 12 hours of education per year
Work Environment:
  • Occasional exposure to unclean or unsanitary conditions during course of home visits including cats, dogs and smokers.
  • Requires daily travel by automobile in all weather conditions
Physical Demands:
Ability to lift fifty (50) pounds. Moving, lifting or transferring of clients may involve lifting to 100 pounds. Ability to exert maximum muscle force to lift, push, pull or carry clients. Ability to use abdominal and lower back muscles over time without fatigue. Ability to bend, stoop, stretch, squat twist, sit and reach. Fine motor skills and visual acuity. Pay:
$23.00 - $26.00 per hour Expected hours: 20.0 - 40.0 per week
