Responsibilities Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and observe patients' conditions closely, documenting vital signs, changes in health status, and medical records accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications safely following prescribed schedules and protocols, ensuring compliance with HIPAA regulations to protect patient privacy. Support clients with specialized needs including dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary; maintain cleanliness by managing laundry and light housekeeping tasks to ensure a safe environment. Communicate effectively with healthcare teams regarding patient status, updates on medical documentation, and adherence to care plans while maintaining professionalism and confidentiality. Requirements Valid CNA certification or relevant caregiving experience in home health or long-term care settings. Proven experience working with seniors or individuals with disabilities . Ability to operate EMR/EHR systems efficiently for documentation purposes; familiarity with HIPAA guidelines is essential. Certifications in CPR and first aid are required; additional training in hospice care or specialized medical procedures is a plus. Strong patient observation skills coupled with compassionate communication; ability to assist with ADLs safely and effectively.
