• About the

Role:

• As a CNA - Day Shift, you will be responsible for providing direct patient care to our residents in our skilled nursing facility.

You will work closely with our interdisciplinary team to ensure that each patient receives individualized care and attention.

Your duties will include:

• Assisting patients with daily living activities, such as bathing, dressing, and grooming
• Providing vital signs and monitoring patients' conditions
• Documenting patient information and progress
• Assisting with pain management and wound care
• Maintaining a clean and safe environment for our patients

• Responsibilities:

• Provide excellent patient care and service, as well as maintain a patient-centered approach
• Work collaboratively with the interdisciplinary team to develop and implement individualized care plans
• Participate in quality improvement initiatives and contribute to a culture of continuous learning and growth
• Communicate effectively with patients, families, and healthcare professionals
• Maintain accurate and detailed records of patient care and treatment
