Job Description
Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Caregiver, where your dedication will make a meaningful difference in the lives of our patients and clients. In this vital role, you will provide personalized, high-quality care to individuals across various settings, including assisted living facilities, nursing homes, and home healthcare environments. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes promptly to healthcare professionals Support with medical procedures such as catheter care, medication administration, and wound care following established protocols Document patient information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; maintain strict confidentiality in accordance with HIPAA regulations Provide companionship and emotional support to patients with dementia or Alzheimer's disease; implement memory care strategies and engage in therapeutic activities Assist with patient transfers utilizing proper methods and equipment like Hoyer lifts or transfer belts to ensure safety for both patient and caregiver Support end-of-life care with compassion, respecting patients' wishes and providing comfort during hospice or palliative care situations Skills Proven experience working with geriatric patients and individuals with disabilities or developmental challenges Strong knowledge of medical terminology, home health care regulations, and safety standards including first aid and basic life support (CPR) certifications Skilled in patient observation techniques to detect changes in condition or behavior; adept at communicating effectively with patients, families, and healthcare teams Familiarity with assisted living environments, long-term care facilities, hospice care, and home health agency operations Ability to perform heavy lifting safely; experience using patient transfer methods and equipment such as Hoyer lifts or gait belts Competent in maintaining accurate medical records and documentation; proficient in using EMR/EHR systems for charting and reporting Excellent caregiving skills combined with a compassionate approach to providing personal care assistance and emotional support Join us in delivering exceptional patient-centered care that enriches lives!
