In this vital role, you will provide high-quality, personalized care to our clients in various settings, including home health, assisted living, and long-term care facilities. Your energetic approach and attention to detail will ensure residents' comfort, safety, and well-being. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse needs, including geriatric patients, those with developmental disabilities, dementia, or end-of-life care requirements. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts. Administer medications following medical documentation protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality standards.
MUST HAVE EXPERIENCE WITH OSTOMY BAG!
Qualifications Valid CNA certification or PCA license with current state registration. Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred. Demonstrated knowledge of medical terminology and medical documentation procedures. Experience working with individuals with disabilities or developmental challenges is highly desirable. Proficiency in EMR systems and familiarity with home health agency operations. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Certifications in CPR and First Aid are required; Basic Life Support (BLS) certification is a plus. Strong interpersonal skills with the ability to communicate effectively with patients of all ages and backgrounds.
