In this vital role, you will provide personalized patient care, assist with activities of daily living (ADLs), monitor health status, and ensure a safe, comfortable environment for residents across various care settings. Responsibilities Assist residents with personal care tasks such as bathing, dressing, grooming, and toileting to promote comfort and dignity. Monitor patients' vital signs, observe changes in condition, and report findings to nursing staff promptly. Support medication administration following prescribed protocols while maintaining compliance with HIPAA regulations to protect patient confidentiality. Help residents with mobility and transfers using proper techniques and equipment like Hoyer lifts and patient transfer methods to ensure safety. Prepare nutritious meals and assist with meal service, ensuring dietary needs are met according to care plans. Maintain a clean, safe environment by performing light housekeeping duties, including cleaning resident areas and common spaces. Support residents during activities of daily living (ADLs), including assisting with ambulation, manual handling, and personal transportation needs. Follow all safety protocols including first aid procedures, CPR certification requirements, and emergency response plans to ensure resident safety at all times. Engage in meaningful communication with clients to foster trust and understanding while respecting residents' rights and preferences. Assist with specialized care such as dementia or Alzheimer's care, memory care, hospice support, or working with individuals with developmental or neurodevelopmental disabilities. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration accurately. Experience supporting residents with Alzheimer's disease, dementia, memory care needs or behavioral health challenges such as autism or intellectual disabilities. Knowledge of HIPAA regulations related to patient privacy and confidentiality is essential. Familiarity with care plans development and implementation for long-term or short-term care settings. Proven ability to handle physical tasks involving heavy lifting, manual handling, and patient transfers safely.
