Job Description

Job Overview Join our compassionate care team as a Caregiver / PCA / CNA, where your dedication and skills will make a meaningful difference in the lives of individuals requiring personalized support. This role offers an energetic environment focused on delivering exceptional patient care across diverse settings, including Home Care. You will be vital in providing hands-on assistance, ensuring safety, and promoting independence for those in your care. Your proactive approach and empathetic attitude will help create a nurturing atmosphere that fosters trust and well-being. Responsibilities Assist patients with daily activities such as bathing, dressing, grooming, mobility, and feeding to promote comfort and independence. Support patients with specialized needs including Alzheimer's care, dementia care, memory care, behavioral health challenges, and working with individuals with disabilities. Use medical equipment such as Hoyer lifts to provide safe and effective assistance. Document all patient interactions thoroughly in medical records while maintaining confidentiality per HIPAA standards. Experience Home Care Prior experience working in diverse healthcare settings such as hospitals (including ICU, emergency medicine), nursing homes, assisted living facilities, or home & community care environments. Experience caring for individuals with developmental disabilities or cognitive impairments such as Alzheimer's or dementia. Familiarity with advanced clinical environments such as Level II Trauma Centers or Level I Trauma Centers is a plus. Strong caregiving background encompassing post-acute care, outpatient clinics, urgent care settings, or hospice services is highly valued.
