Responsibilities:

• Provide direct patient care under the supervision of a registered nurse or licensed practical nurse
• Assist with activities of daily living, including bathing, dressing, and feeding
• Administer medications as directed by healthcare professionals
• Monitor and record vital signs, such as blood pressure and temperature
• Assist with tube feeding and catheterization as needed
• Provide emotional support to patients and their families
• Collaborate with the healthcare team to develop and implement care plans
• Document patient information accurately and timely in electronic health record systems, such as eClinicalWorks
• Follow infection control protocols to maintain a safe and clean environment

Requirements:

• Valid certification as a Nursing Assistant or completion of a Nursing Assistant training program
• Experience in hospice care, behavioral health, senior care, or working with people with developmental disabilities is preferred
• Knowledge of medication administration procedures and safety precautions
• Ability to effectively communicate with patients, families, and healthcare professionals
• Strong attention to detail and ability to prioritize tasks
• Physical stamina to perform duties such as lifting and transferring patients
