In this rewarding position, you will support patients with their daily needs, monitor health conditions, and promote their overall well-being in a nurturing environment. Duties Assist residents with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using proper patient transfer methods like Hoyer lifts and manual handling techniques. Monitor and record vital signs, observe patient conditions closely, and report any changes to nursing staff promptly to ensure optimal care. Support medication administration under supervision, ensuring compliance with HIPAA regulations and resident rights. Prepare nutritious meals and assist with meal service while maintaining cleanliness in dining areas to promote health and safety. Provide companionship and emotional support to residents, fostering a positive environment that respects individual preferences and dignity. Assist with light housekeeping tasks including cleaning resident rooms, common areas, and ensuring infection control standards are maintained. Skills Proven experience working in home health environment. Strong patient observation skills with the ability to recognize changes in condition or behavior related to dementia, Alzheimer's disease, autism, or other neurodevelopmental disorders. Knowledge of care plans, medication administration procedures, and resident's rights within healthcare settings. Ability to perform first aid and CPR confidently; trained in basic life support techniques for emergency situations. Excellent communication skills for effectively engaging with residents, families, and healthcare team members. Physical stamina for heavy lifting, patient transfers using Hoyer lifts or other assistive devices, and manual handling tasks required for patient safety. Familiarity with HIPAA compliance standards to ensure confidentiality of patient information at all times. Experience supporting individuals with developmental disabilities or intellectual disabilities across diverse care environments is an added advantage.
