Job Summary We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Personal Care Assistants (PCAs) to join our team in providing exceptional patient-centered care. In this vital role, you will support individuals across various care settings, including assisted living, nursing homes, home healthcare, and hospice environments. Your energetic approach and attention to detail will ensure that residents and clients receive the highest quality of care, promoting their comfort, safety, and well-being. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; document medical information accurately in Electronic Health Record (EHR) systems while maintaining strict confidentiality per HIPAA regulations. Support medication administration under supervision, ensuring proper handling, storage, and documentation of medications following home healthcare regulations. Provide specialized care for individuals with dementia or Alzheimer's disease by implementing memory care techniques and engaging patients in appropriate activities. Assist with catheter care, wound care, and other medical procedures as trained; respond promptly to patient needs during end-of-life or hospice care situations. Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning patient areas, and meal preparation to promote overall health and comfort. Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care delivery; document all interactions thoroughly in medical records. Skills Strong knowledge of medical terminology, patient observation techniques, and home health care regulations. Experience working with geriatric patients or individuals with developmental disabilities; familiarity with dementia and memory care is highly valued. Proficiency in using EMR/EHR systems for documentation; understanding of HIPAA compliance to maintain patient confidentiality. Ability to assist with ADLs efficiently while demonstrating compassion and patience; experience with patient transfer methods such as Hoyer lifts or other mobility aids. Knowledge of basic life support (BLS), first aid procedures, CPR certification preferred; ability to respond calmly during emergencies. Physical stamina for heavy lifting tasks; ability to work independently or as part of a team in various settings including assisted living facilities or home environments. Excellent communication skills for client interaction; adept at caregiving tasks such as meal preparation, light cleaning, and providing emotional support to patients and families.
