Prairie Hearts Home Care LLC is seeking a compassionate, dependable, and professional Certified Nursing Assistant (CNA) to provide personalized in-home care to clients in the Bismarck/Mandan area and surrounding communities. The CNA will assist clients with activities of daily living while promoting independence, dignity, comfort, safety, and quality of life.

Key Responsibilities:

Assist with bathing, dressing, grooming, oral hygiene, and other personal care needs. Assist with toileting and incontinence care. Assist clients with mobility, transfers, positioning, and ambulation. Provide feeding assistance and help with meal preparation as needed. Provide routine catheter and ostomy care within the CNA's permitted scope and training. Measure and document intake and output when included in the care plan. Take and document vital signs when assigned and permitted. Provide medication reminders; CNAs will not independently administer medications unless appropriately authorized and permitted. Provide companionship, conversation, supervision, and emotional support. Assist with light housekeeping, laundry, changing bed linens, and maintaining a safe client environment. Assist with errands, grocery shopping, appointments, and approved transportation when assigned. Observe clients for changes in condition and promptly report concerns to the appropriate supervisor or nurse. Follow each client's individualized care plan and Prairie Hearts Home Care LLC policies and procedures. Accurately document services provided during each visit. Maintain client privacy and confidentiality. Follow infection-control, safety, emergency, and incident-reporting procedures. Treat every client and family with kindness, dignity, respect, and professionalism.

Qualifications:

Applicants should have a current CNA certification appropriate for the position, CPR/First Aid certification if required by the agency, and the ability to safely perform the physical duties of home care. Candidates should have strong communication skills, reliable attendance, and the ability to work independently in clients' homes. A valid driver's license, reliable transportation, and acceptable driving record may be required for positions involving client transportation. Employment may also be contingent upon required background checks and other applicable North Dakota home-care requirements.

Work Environment:

This position involves traveling to clients' private homes and providing one-on-one care. Hours may include weekdays, evenings, weekends, holidays, and varying shifts depending on client needs.
