Job Description

Join RiverWoods Exeter as an LNA/MNA providing compassionate evening care to residents in our continuing care retirement community. In this full-time role, you'll assist with daily living activities, administer medications per certification, monitor resident conditions, and support a safe, respectful environment. You'll collaborate closely with nurses and interdisciplinary team members to promote dignity, comfort, and wellness. Responsibilities Provide hands-on assistance with activities of daily living, including bathing, dressing, grooming, toileting, and mobility for senior residents Support residents with safe transfers, ambulation, and use of assistive devices, following proper body mechanics and safety protocols Monitor and record vital signs and other basic clinical observations, reporting changes in residents' conditions to licensed nursing staff Administer medications within LNA/MNA scope of practice and state regulations, following established medication administration procedures Maintain accurate and timely documentation of care, observations, and medication administration in accordance with facility policies Respond promptly and courteously to resident call lights and requests, providing compassionate, person-centered care Assist with mealtime support, including setting up trays, feeding assistance as needed, and encouraging hydration and adequate nutrition Promote infection control and safety by following established procedures for hand hygiene, personal protective equipment, and environmental cleanliness Participate in shift change reports and communicate effectively with nurses, therapists, and other team members to ensure continuity of care Build supportive relationships with residents and their families, demonstrating respect, patience, and empathy in all interactions Help maintain a clean, organized, and homelike environment in resident rooms and common areas Support residents' evening routines and bedtime preparation during the 3p-11p shift, providing comfort and reassurance Required Skills Patient care assistance Vital signs measurement Medication administration (MNA) Electronic health records (EHR) documentation Infection control practices Safe patient transfer and mobility Basic wound and skin care observation Resident safety monitoring Communication with residents and families Interdisciplinary team collaboration Time management Recognizing and reporting changes in condition HIPAA compliance Basic computer skills
