Job Description

Oceanside Center, a Genesis HealthCare skilled nursing and rehab facility in Beverly, MA, seeks a Wound Care and Skin Health Team Lead, RN. In this leadership role, you will oversee wound and skin integrity programs, perform complex assessments, deliver evidence-based treatments, and coordinate care with an interdisciplinary team. You will educate and mentor staff, support quality improvement initiatives, ensure regulatory compliance, and champion a patient-centered approach that upholds dignity, safety, and optimal healing outcomes for our residents. Responsibilities Lead and coordinate wound care and skin integrity programs for residents Perform comprehensive wound assessments and develop individualized care plans Provide direct clinical wound care, treatments, and dressing changes Collaborate with physicians, therapists, and interdisciplinary team on care plans Educate nursing staff on wound prevention, treatment protocols, and documentation Monitor skin health metrics, track outcomes, and report trends to leadership Ensure compliance with regulatory standards and best-practice guidelines Coach, mentor, and support nursing staff in wound and skin care practices Communicate with residents and families regarding wound status and care goals Participate in quality improvement initiatives related to wound and skin health Required Skills Wound assessment and staging Advanced wound care treatments and dressings Pressure injury prevention and management Infection control and aseptic technique Care plan development and documentation Electronic health record (EHR) charting Interdisciplinary care coordination Staff education and clinical mentoring Regulatory and compliance knowledge (LTC/Skilled Nursing) Quality improvement in clinical settings
