Responsibilities:

Provide direct patient care and support in accordance with your licensure and certification level Assist patients with activities of daily living (ADLs) including bathing, grooming, and mobility Monitor and document patient health status, vital signs, and changes in condition Administer medications and treatments as directed (RN/LPN) Collaborate with care teams to develop and implement individualized patient care plans Offer emotional support and companionship to patients and their families Maintain a safe, clean, and comfortable environment for patients

Requirements:

Active and valid RN, LPN, HHA, PCA, or CNA license or certification in New Jersey Prior experience in home health, assisted living, or clinical care preferred Strong communication and interpersonal skills Ability to work independently and as part of a collaborative care team Reliable, punctual, and committed to delivering high-quality patient care Compassionate, patient-centered approach to caregiving Ability to pass a background check and meet all state compliance requirements
