Job Description
About the Role TruCare Management is seeking motivated and detail-oriented Certified Nursing Assistants (CNAs) to join our growing Chronic Care Management (CCM) team. This is a non-clinical, in-office position focused on coordinating care for patients with chronic conditions. You will play a key role in improving patient outcomes by maintaining consistent communication, tracking health data, and supporting providers with documentation and care planning.
