Job Description

The duties below outline the general responsibilities associated with this position. Each of these duties is considered critical to effective department operations and our Mission. It is expected that all staff in this position will have satisfactory attendance and will perform these duties as assigned in a competent, cooperative, and timely manner. The Job Description will be reviewed periodically for accuracy and/or adjusted according to business necessity. 1 Answers call lights and responds to residents' verbal requests to meet residents' needs in a timely and polite manner. 2 Identifies and reports clothing needs to the nurse: lack of items, repair of torn or ripped items, replacement needs. 3 Ensures the use of eyeglasses, hearing aids and dentures as appropriate to specific residents. Reports missing/non-functioning eyeglasses, hearing aids and dentures immediately to nurse. 4 Ensures the proper use of prosthetic devices, assistive devices and adaptive equipment as appropriate. 5 Communicates in a dignified manner with all residents, including those unable to respond appropriately, 6 Demonstrates an understanding of the interests and needs of the geriatric resident and treats the residents with kindness and respect, observing Resident Rights. 7 Maintains a restraint-free environment for all residents unless deemed necessary by physician and care plan. 8 Provides a safe environment and employs all precautions to prevent resident, personal, and staff injuries. 9 Provides privacy for the resident at all times by closing cubicle curtains and doors; using bed linen and lap robes, knocking on doors to resident rooms, bathroom areas and introduce self. 10 Assists with admission, discharge, transfer and post mortem care of residents. Always maintains sensitivity and dignity towards resident. 11 Interacts with residents' families in a cooperative, friendly and helpful manner and refers them to the appropriate source for information concerning the residents' care or condition. 12 Has general knowledge of advance directives/health care proxy/DNR. 13 Conducts self in an ethical manner and maintains confidentiality of resident information at all times both within and outside the facility. 14 Provides and assists with all aspects of personal hygiene; partial bath, bed bath, whirlpool tub bath, shower, shampoo, oral hygiene, shaves, and nail care. Encourage resident involvement in care by utilizing task segmentation. 15 Provides and assists with all aspects of dressing and undressing: appropriate dresses, pants, skirts, sweaters, under garments, shoes, socks, stockings, hair combing, jewelry, and make-up. Encourage resident involvement in care by utilizing task segmentation 16 Meets the toileting needs of the residents including answering toileting requests promptly, assisting on and off toilet or bedpan, providing a urinal, emptying urinals and bedpans promptly, changing disposable under garments, and performing perineal care. Replaces toileting equipment when needed. 17 Provides skin care by washing, drying, lubricating, and observing for any changes in the skin; reports changes in the skin immediately to the nurse. 18 Assists with meeting the nutritional needs of the residents by: preparing them for meals; encouraging completion of, assisting with, and/or feeding the meal; passing tray; offering alternate if needed; providing adequate hydration; providing nourishments; reporting consumption. Be aware of planned seat arrangement. 19 Assists with or transfers residents from bed to chair or to a standing position. Uses Hoyer lift with assist to transfer dependent residents as per policy and procedure. Seeks out current information regarding residents' current plan/transfer ability. 20 Transports residents via chair or stretcher from one area to another. Seeing that they are properly groomed, positioned and dressed. 21 Assists with or ambulates residents according to Rehab recommendations and floor ambulation program. 22 Carries out a bowel and bladder training program as prescribed for specific residents. 23 Assists with or performs range of motion as prescribed for specific residents. 24 Encourages orientation of the resident to person, time and place. 25 Turns/positions resident as per established schedule and documents same. 26 Assist in other activities as indicated. Minimum Requirements The following educational minimum requirements must be met by all incumbents: General Must possess professionalism, strong organizational and interpersonal skills, have a polished presentation and be able to maintain strict confidentiality on all matters. Must be self-motivated, able to work with minimal supervision. Education Current New York State Certification. Experience Preferably two (2) years experience in long term care.
