Job Summary OwnaHealth is a virtual clinic on a mission to reverse Type 2 Diabetes and obesity through food, not pharmaceuticals. We serve underserved populations — people with limited access, limited means, and often very complicated cases of uncontrolled diabetes. Using a nutrition-first approach grounded in high-fat, low-carb and ketogenic principles, we help patients eliminate insulin use, reduce A1c levels by over 2 points, and regain control of their lives — even in the most challenging circumstances. Reverse the diabetes and obesity epidemic: Help solve one of healthcare's biggest challenges by using food as medicine, not just prescriptions. Practice deprescribing, not just prescribing: Get patients off insulin, sulfonylureas, and the long list of medications they've been stacked on for years — and see the results. Full-time, in-clinic role: Work in-person from one of more than 50 office locations across New York State, upstate and downstate, role starting in late 2026.
Pay and benefits:
$110,000-$130,000 per year, plus great health insurance and PTO.

We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant (Endocrinology) to join our innovative healthcare team. In this role, you will deliver expert endocrine care through Owna's AI-powered mobile app, providing comprehensive assessments, diagnosis, and management for patients with hormonal and metabolic disorders. Your expertise will help improve patient outcomes through in-person visits, education, and collaborative care planning. This position offers an exciting opportunity to leverage your clinical skills in a technology-driven environment dedicated to accessible, high-quality healthcare while also flexing non-clinical muscles if you desire a leadership role within Owna Responsibilities Provide endocrinology care to patients from one of more than 50 office locations across New York State, through by our AI-powered Mobile App (Telemed Visits, however you'll need to be on-site at an office( Manage a small caseload of medically complex patients with uncontrolled Type 2 Diabetes, obesity, and related cardiometabolic conditions. Lead medication management and deprescribing — including titration and discontinuation of insulin, oral antihyperglycemics, anti-hypertensives, and other comorbidity medications as patients improve. Order, interpret, and act on labs, CGM data, and remote monitoring data (BG meter, scale, BP cuff, ketone meter) to guide clinical decisions. Develop and adjust individualized treatment plans rooted in low-carbohydrate and ketogenic nutrition protocols. Collaborate closely with care coordinators, coaches, dietitians, and supervising endocrinology to ensure comprehensive care. Quickly establish rapport and therapeutic alliances with a diverse, often underserved patient population. Utilize motivational interviewing and other evidence-based techniques to support sustained behavior change. Consistently meet clinical and program metrics while maintaining high-quality, compassionate care. Complete all clinical documentation in a timely manner. Qualifications Active Nurse Practitioner (NP) or Physician Assistant (PA) licensure in the State of New York, in good standing. Spanish Fluency Clinical experience managing Type 2 Diabetes, obesity, and cardiometabolic comorbidities. Strong interest in low-carbohydrate / ketogenic nutrition, lifestyle medicine, and medication deprescribing. Experience and/or genuine interest working with underserved or high-risk populations. A compassionate, non-judgmental, and empowering approach to patient care. Comfort with electronic health records and digital health tools.

experience with eClinicalWorks (eCW). Demonstrated ability to adapt communication techniques to meet the needs of a diverse patient population. Ability to work full-time, in person, from an office locations in upstate or downstate New York.
Pay:
$110,000.00 - $125,000.00 per year
