Job Description

Lifecare Family Health Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) to join our outpatient team serving patients across primary care and specialty services. This role provides nursing support, patient care, clinical assistance, and care coordination in a fast-paced healthcare setting serving individuals and families throughout our community. As a Federally Qualified Health Center, Lifecare provides patient-centered medical, dental, vision, and behavioral health services. Responsibilities include patient rooming, obtaining vital signs and health information, assisting with procedures, patient education, point-of-care testing, follow-up coordination, documentation, and general support of clinic operations. LPNs and CMAs may support multiple service areas based on operational needs, including primary care, specialty services, patient follow-up, and other clinical support functions. Key Responsibilities Welcome, room, and prepare patients for provider visits Obtain and document patient histories, vital signs, and other clinical information Communicate patient concerns, needs, and changes in condition to providers Assist providers during examinations and procedures Provide patient education and review discharge instructions Perform clinical procedures and treatments within LPN or CMA scope of practice and organizational policy Complete point-of-care testing and support laboratory documentation as assigned Support patient follow-up, scheduling, and coordination of care Maintain accurate documentation in the electronic health record Prepare and stock exam rooms, equipment, medications, vaccines, and supplies Follow HIPAA, OSHA, CLIA, infection control, and safety requirements Travel to other Lifecare locations as operational needs require Perform other duties as assigned Additional Clinic Support Responsibilities Depending on clinic needs, LPNs CMAs may also assist with specialty support and care coordination activities such as: Vision/optometry clinic support Insurance verification and patient scheduling assistance Follow-up support and scheduling after hospital or emergency room visits Coordination of follow-up appointments and documentation in the EHR Additional patient support activities related to continuity of care and value-based care initiatives Qualifications Required Current unrestricted LPN license in the State of Ohio Graduate of an accredited school of nursing BLS certification within 90 days of employment Strong communication, organizational, and documentation skills Proficiency with electronic medical records and standard office technology Preferred Previous outpatient clinical experience Experience in primary care, specialty care, or community health settings Experience with patient education, follow-up coordination, or care management Benefits Health insurance Dental insurance Vision insurance Health savings account Life insurance Employee assistance program Paid time off Retirement plan Employee discount Lifecare Family Health Center is committed to providing high-quality healthcare services while fostering a respectful, inclusive, and supportive workplace for employees and patients alike.
Pay:
$23.50 - $25.00 per hour
