Job Description

Overview We are seeking compassionate and dedicated experienced Home Health Aides (HHA), State Tested Nursing Assistants (STNA), and Certified Nursing Assistants (CNA) to join our dynamic caregiving team. In this vital role, you will provide exceptional patient-centered care, ensuring comfort, safety, and dignity for individuals across various care settings. Your expertise will help improve quality of life for clients with diverse needs, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care. Duties Deliver personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, assisting with medication administration, catheter care, and documenting medical information accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support patients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement tailored to their needs. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe and hygienic environment in compliance with home healthcare regulations. Communicate effectively with clients and their families to provide updates on patient condition while maintaining strict confidentiality in accordance with HIPAA standards. Follow all safety protocols including first aid, basic life support (BLS), CPR certification, and emergency response procedures to ensure patient safety at all times. Facilitate patient transfers using appropriate methods and equipment while adhering to proper body mechanics to prevent injury. Qualifications Valid certification as an HHA, STNA, or CNA from an accredited program; OR 1 year verifiable experience is highly preferred. Demonstrated experience working with geriatric patients, individuals with developmental disabilities, dementia or Alzheimer's disease, hospice or end-of-life care. Knowledge of medical terminology, medication administration procedures, catheter care, vital signs monitoring, and patient personal care assistance. Familiarity with home health agency operations, home & community care environments, EMR/EHR systems, and documentation standards such as medical records management. Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts; physical stamina is essential. Strong communication skills for client interaction and team collaboration; ability to maintain patient confidentiality at all times. Certifications in first aid and CPR; knowledge of home health care regulations is a plus. Experience working with individuals with disabilities or in long-term care settings is advantageous.
